Dementia Care
Tailored dementia care at home
Dementia is a medical condition that affects the memory and brain. Each person’s experience with dementia is unique and the symptoms present differently and progress at different speeds. These differences are linked to which parts of the brain are affected and the type of disease that is causing the dementia.
Caring for a parent or loved one living with dementia is a complicated task and there isn’t a “one size fits all” approach on how it should be managed.
The experience of living with dementia is confusing and complicated and building a care package based around each client is vital. Often the most effective care for dementia at home is based around routine as this helps our clients feel secure in their own homes.
Using our online PASS software to manage the administration of your care, through to the family being able to log on and read the care notes from anywhere in the world, means families are constantly up to date on their loved ones health and well-being.
Care for dementia at home with a difference
As well as the more traditional aspect of managing dementia care at home, our tailored care packages take into consideration a holistic approach. This includes diet, exercise and keeping you in touch with your hobbies and activities.
Whether it’s helping with a jigsaw or organising a paper delivery, these small but important factors help keep you happy and in touch.

Sarah's Story
Ealing resident having dementia care at home
Sarah was diagnosed with Alzheimer’s in 2019 and has support from Care Concern to continue living independently at home where she has lived for over 50 years. Her family were concerned when Sarah started missing meals due to short-term memory issues.
Initially Sarah had two companionship visits per week from Care Concern to go for a walk but this slowly increased over time as she got to know her care team. Sarah is very involved in decisions about her care and now has three daily support visits for her personal care and meals and to engage Sarah in her favourite activities.
Her care visits enable her to live safely at home and her family can relax, knowing that Sarah’s needs are met. When they visit each week, they are able to enjoy their time together.
